Where can a find the best software for a questionnaire template?
Free or for a fee, I need software that I can tool around with for adding questions and print them in a nice, easy-to-read format. I want to be able to print this off or e-mail it to customers so that it looks highly professional and is easy to edit/use. Thank you!

BUT.
How about using your own word processor (eg Word 2000).
You can open up a new document and bring up "Table" and customise it.
It is really easy.
